I am having a similar problem with a SOP Blank History report. For some reason, I am unable to display the "column headers" (coming from the page header section in report writer) on any page other than page 1 (aka report header).
I've recreated the report both in Report Writer as well as in the Report Template but can't seem to get pages to display the column information on pages 2-.... I have modified the similar Work (unposted) version of this report and it works fine.
I am interested in any suggestions that arise from this thread, as well.
